Hi bros....

Has anyone tried shockwave therapy for erectile dysfunction? Can share your experience?

Now is available over at public hospitals and i am on my way to a polyclinic today to get a referral letter to SGH.

It's not a magic bullet I tried it out and results not conclusive for me.

However I was not an ED sufferer in the true sense of the word because mine was still functional but not as hard as it was when I was in my 20s.

For someone who can't get it up at all or who cannot keep it up long enough to complete the act it might work better.

If you have money to burn no harm in trying otherwise ED drugs might be cheaper even in the long run especially now that generic Viagra is on the market at very reasonable prices.

Have you talked to private clinics vs going to hospital, whether in terms of pricing, how much is the difference?

As far as I know, private clinics is around 200-400 per time and around 6 times per treatment?

I planning to go as I think my diabetes is making my ED very obvious at times....taking Levitra helps but its too not...spontaneous leh....so hoping this shockwave thingy can help abit... if 1000+ to 2000+, then not too expensive to trial....

Dear guys,
I have just gone for my 1st session of shockwave therapy this morning, at i.medical at north point city. It cost 199 per session with one time consultation at 50 dollars.

Before this, I was referred to SGH for the shockwave through a polyclinic. Total cost at SGH is 1500 for 6 sessions. It is more expensive than the private clinic at north point city. Thats why i decided to go for the one at north point city. Furthermore over at SGH, there is a waiting time of about 2 months to start the treatment.

Tried before for 6 sessions at Northpoint and initially like have some results but wear off pretty fast. Results not as hard as I would like it to be. Sadly ED seems to be bad, always go soft quickly. Relying bk on drugs

From my initial research, sometimes it is meant to be complement the ED drugs you are taking, to make it even more effective. For some reasons, Levitra can be quite ineffective for me in the afternoon...not sure why....so maybe with this therapy it may only help in this way...

199 per session is not too expensive to trial, so I might just go when I have the time..... but I also seen some articles saying some clinics use machines that are not so called certified, so the machines they use might make a difference in the price?

Debate continues about use of shockwave therapy to treat ED

The potential for using low intensity shockwave therapy to treat erectile dysfunction (ED) was first proposed in 2010, and early studies have suggested that it may provide some benefits. However, the ideal energy levels and frequency of treatment are a matter of debate.

Two experts took up that debate during Sunday morning’s Prime Time plenary session in a 20-minute presentation moderated by Ranjith Ramasamy, MD, assistant professor and Director of Reproductive Urology at the University of Miami.

“Current ED treatment options include PDE5 inhibitors (PDE5-i), vacuum erection devices, intracevernosal injections, intraurethral suppositories and penile implants,” said Dr. Ramasamy, setting the stage for the debate. “These treatments attempt to improve erectile function without treating the underlying pathophysiology of ED, leaving us with the question as to whether we can actually heal the penis.”

Shockwave therapy for treating erectile dysfunction, he noted, is quite different from extracorporeal shockwave lithotripsy (ESWL®) used to treat kidney stones.

“ESWL has a smaller focal volume and the energy is concentrated, as opposed to ED, where the shockwaves are radial, the focal volume is larger and the energy is spread over a greater area,” Dr. Ramasamy explained. “In fact, the energy is about 10 percent of what we use for ESWL for kidney stones.”

The case in favor of shockwave therapy for ED was presented by Irwin Goldstein, MD, Director of Sexual Medicine at Alvarado Hospital, clinical professor of Surgery at the University of California, San Diego, Director of San Diego Sexual Medicine and Editor-in-Chief of Sexual Medicine Reviews.

“So, why are we looking at shockwave therapy when we already have everything we need to treat erectile dysfunction?” he asked. “We have pills now that are amazing, although they are not for everyone. They are contraindicated, for example, in people who can’t walk up three flights of stairs. Additionally, the pills don’t work in about 30 percent of people, and half of the people discontinue therapy. So, the search for additional therapies is ongoing.”

While traditional ED therapies are effective at treating the symptoms of ED, they do not change the underlying pathophysiology of the condition.

“Through shockwave therapy, we’re introducing a new paradigm of disease modification to the treatment of ED,” Dr. Goldstein said. “You can take a pill, but you wake up the next day with the same erectile tissue and the same problem. If you undergo shockwave therapy, what we hope it will do is change the health of the tissue. Injections, pills or implants don’t really address pathology reversal, but low intensity shockwave will.”

Although shockwave therapy for treating ED has not received approval from the U.S. Food and Drug Administration, Dr. Goldstein said he is involved in a clinical trial that he hopes will change that.

“I think we have a lot of good clinical data to support shockwave therapy in humans,” he said. “We are now involved in a randomized double-blind, placebo controlled, open label extension of an earlier pilot study we did. We believe this is a revolutionary therapy and we hope that the data we present in the future will get us across the finish line and get this therapy approved.”

Arguing against the use of shockwave therapy for ED, Tom Lue, MD, professor and Vice Chair of Urology at the University of California, San Francisco, said he believes there are still too many unanswered questions regarding the optimal energy range, frequency of treatment and long-term effects that need to be answered before abandoning traditional treatment strategies.

“In shockwave therapy, the energy level is very important. At a certain level, you may have a beneficial effect. But if you gradually go higher, it’s less and less effective,” he said. “In fact, a 2008 study in animals showed that at higher levels, animals actually become impotent and it causes fibrosis. So, there’s some concern that, unless you know the ideal energy level and unless you can calculate the biologic effect, it’s not going to work and it may actually harm the patient.”

Over at the consultation at SGH, was told of 3 options for erectile dysfunction.

1) A penis pump called Erec Tech System, costs about 750 dollars from the local distributor. But u can get it at about 500 plus over the internet.
2) By injection - inject the medication into your penis each time before sex. Can be rather painful.
3) Shockwave therapy

I did some research on the penis pump and found out that those cheap vacuum penis pumps work the same way as Erec Tech System. Thus, bought one over shopee for less than 20 dollars.

I received it last week and have been using it almost every night for about 20 mins according to those youtube videos on how to use a penis pump.

I used the penis pump last night after my 1st session of shock wave in the morning.

Today, i feel a strong sexual urge in my penis, probably due to the 1st session of shockwave. That's a piece of good news to me.

Viagra gives me a blue vision side effect and a bad headache if i drink beer or other alcoholic drinks....

I have been taking cialis for many years. One good thing about cialis is that it doesn't give me a bad headache with beer or any other alcoholic drinks and it can last much longer than viagra. But as i age, the side effects of running nose and body aching occur more frequently.

That's the reason why i go for shockwave therapy and that's why it was not included in one of the 3 options given to me during the SGH counsultation.

Future Treatments for Erectile Dysfunction

Future treatments for erectile dysfunction focus on providing medications that are more effective, work rapidly, and have fewer, if any, side effects than currently available treatments.

Currently there are five oral drugs available to treat ED -- Cialis, Levitra, Stendra, Staxyn, and Viagra. A number of pharmaceutical companies are researching new treatments for ED, and many new options may be just around the corner. These include:

Uprima: Uprima (apomorphine) comes in a tablet form that dissolves under the tongue. Uprima works by stimulating the brain chemical dopamine, which heightens sexual interest and sensations. Its major side effects are nausea and vomiting. Additionally, a small number of people passed out after taking Uprima. Therefore, its release in the U.S. is on hold. It is currently available in Europe. Clinical trials are also currently being conducted on a nasal spray form of this drug, which may cause less nausea.

Topiglan: Still under investigation, a cream applied to the penis called topiglan uses the same drug (alprostadil) that is injected and also used in suppositories to treat ED. If topiglan proves to be safe and effective, it is still not entirely clear which patients would benefit from its application and whether patients on injection and suppository therapy would no longer have to use these techniques.

Melanocortin activators: These are drugs that appear to act through the central nervous system (for example, the brain). They have been shown in animal studies to produce an erection. Initial studies in humans suggest that the drug (PT-141) can be effective if given intranasally (through the nose) in men with nonmedical (psychological/emotional) rather than physical causes of ED and mild to moderate ED. Larger studies will be necessary to demonstrate the safety and overall effectiveness of these drugs.

Gene therapy: This novel therapy would deliver genes that produce products or proteins that may not be functioning properly in the penile tissue of men with ED. Replacement of these proteins may result in improvement in erectile function. Experimental animal models have demonstrated improvement in erectile function with gene therapy. Human studies may also demonstrate success with this therapy. Gene therapy may take a long time for regulatory approval and public acceptance.
